He occasionally replies to people's tweets though he has something like 15,000+ followers now so he can't read everything everyone sends him. I don't doubt his home page is flooded. If you're signed in to Twitter you should be able to post. I think most of the people leaving the messages on the pictures are just random people on Twitter and not people Stephen knows personally.
